My Italian grandma used to play this all the time by Caruso. She explained that this guy can't tell reality from the play they are acting in ( inside the Opera about actors[clowns] putting on a play) anyway this guy is crying about how his cheating wife is making a clown of him and he kills his wife and " lover". But it is in the play she is pretending to cheat on her husband with his friend. In reality she is faithful. So he has killed his wife and friend because of his insanity. The aria itself always gives me chills.
