1. I worked for a company that made surgical supplies & equipment.

Some workers on the production line claimed the machine wasn't properly sealing the product, not every time but they saw it often enough they reported it. Management went & looked at a dozen or so packages & said everything was fine. Two weeks later our customer service department was bombed with complaints from customers that almost half their surgical products weren't sealed properly.
