I first heard about it when I was watching one of the "Engineering Disasters" episodes on cable TV many years ago.

I was always thankful that nothing I ever worked on ended up making it on that show! My first job was on the Space Shuttle but thank God it was the O-rings on the Challenger that failed which I had nothing to do with. Then when the Columbia came apart during reentry I was deathly afraid that it WAS my part that time but when I read the accident report it turned out that what I had worked on was actually firing like crazy to keep the vehicle pointed in the right direction as the wing melted and kept the crew alive as long as it could until it reached its limit and that was that.

Sent a real chill up my spine reading that report, boy.
